Create Medical Documents

Create Medical Documents

All the medical documents like Medical Certificates, Referral Letters and Mental Health Plans are all stored as Clinical Notes. When creating such types of medical documents you always create a new Clinical Note at first and then apply different Templates for the desired contents. Refer to Guide to clinical notes for more information.

Create a medical document step by step

  1. Navigate to Clinical Notes section. You can quickly access a patient’s clinical notes in two ways:

  2. Open the patient’s profile and click the Clinical Notes tab. Select a clinical note from the list on the left panel to open it.

    Open the patient’s appointment on the calendar, then click the Notes link. This opens the clinical note for this specific appointment.

  3. On the top right, click New Clinical Note.

  4. Tip

    The system will ALWAYS creates a clinical note named “Appointment” at each time an appointment is booked for a patient. It is a good practice to use it as a diary to record any information regarding to the appointment on that day and avoid to use ANY template on it.

  5. In the template editor, click the Templates button.

  6. In the pop-up, you’ll find a list of clinical templates available to you. To preview a template’s contents, click the arrow beside its name to toggle open a preview. You can also use the search bar to find a specific template.

  7. In Doctor@Home we have the following templates for you to choose when you create a medical document:

    • MEDICAL CERTIFICATE

    • REFERAL LETTER

    • MENTAL HEALTHCARE PLAN REVIEW

    • GP MENTAL HEALTH TREATMENT PLAN (MBS ITEM NUMBER 2700/ 2701/ 2715/ 2717) PATIENT ASSESSMENT

    • ENHANCED PRIMARY CARE (EPC) PROGRAM REFERRAL FORM FOR ALLIED HEALTH SERVICES UNDER MEDICARE

    • CHRONIC DISEASE MANAGEMENT 721 GP MANAGEMENT PLAN 723 TEAM CARE ARRANGEMENTS

    • CHRONIC DISEASE MANAGEMENT 732 REVIEW OF GP MANAGEMENT PLAN OR 732 REVIEW OF TEAM CARE ARRANGEMENTS

    • PATIENT INFORMATION TEMPLATE

    • GENERAL DOCUMENT TEMPLATE – DOCTOR@HOME

  8. Choose to use one of the above templates. The contents of the template will be imported into the editor.

    Warning

    By importing a template the ENTIRE content in the current editor will be replaced. Make sure you have other copy of this content if you want to use it in the future.

  9. Replace the contents within the ANGLE BRAKETS ‘<‘ and ‘>’ with the REAL contents, and feel free to insert any other contents desired.

    Warning

    IMPORTANT! DON’T delete or edit the contents in SQUARE BRACKETS ‘[‘ and ‘]’. They are used as Dynamic Terms which will populate the REAL data AUTOMATICLLY when you publish the clinical note.

  10. Click the small downward triangle in the upper right corner and select Save.

  11. Click the small downward triangle in the upper right corner and select Publish.

Note

Until you have the Clinical Note published, all Dynamic Data you imported from the template won’t populate to the REAL value. You need to save and publish the Clinical Note before you print or send it to a patient or other specialists.

Note

Saving or Publishing a Clinical Note will NOT send it to your patient or other specialist automatically. If you want to send for example a Medical Certificate to the patient, you need MANUALLY to send it via sending an email. See SHARE YOUR CLINICAL NOTES for reference.
